Each patient is admitted under the care of an attending physician. Board-Certified Specialty Physicians, such as Cardiologists, Pulmonologists, Nephrologists, Surgeons, and Infectious Disease doctors are available for consultation. At Apex, they understand that every patient has individual needs. A personalized plan of care is developed through an interdisciplinary team approach, treating the individualized needs of every patient. The patient’s family is encouraged to participate and kept informed of the treatment plan and goals. Apex referrals may be received by healthcare professionals, family or friends. Once an order is received from your physician, an on-site assessment will be performed by a clinical liaison/nurse within two hours.
